PURCHASE AGREEMENT

This puppy is guaranteed to be in good health to the best of our knowledge at
the time of this sale and has been examined by our licensed veterinarian. This
puppy has had it's FIRST inoculation against distemper, hepatitis and
parvovirus, we do NOT give the Lepto, your vet can do that when your puppy
gets his/her booster does not mean that a puppy is immune to these diseases.
Therefore you, the buyer are fully responsible to maintain your puppy's
continued vaccinations and to do so on a yearly basis according to your vet's
recommendations. Puppies need 3 shots about 3 weeks apart. 3 sets are all
that is needed. I also guarantee the puppy for one year from date of birth
against any hereditary disorder that may interfere with the puppy's ability to
lead a normal life. Hip or elbow dysphasia will not be based on x-ray only, the
dog must demonstrate pain and clinical signs of the disease to be considered
under the one year guarantee. There will be no replace men t of dogs that are
susceptible to any of the above that are overweight or overexercised. The
purchaser must agree to keep the pup on a good high premium dog food.
Luxating patellas must be severe enough to hamper the pup's ability to lead a
normal life. If the dog should die within this one year guarantee, purchaser
will pay to have it autopsied to ascern reason. If reason is genetic Indian Creek
will replace the puppy out of the next available litter. Under no circumstance
will cash be refunded. In the event a replacement puppy is given, all shipping
costs are the responsibility of the buyer. Buyer agrees and fully understands
that this contract is in jurisdiction of the State of Oklahoma in the county of
Woodward.
It is agreed that any litigation, will take place in the Woodward County
Oklahoma. As a buyer, you have three (72 hours) business days, from the date
of purchase, to have this puppy ex-amined by your own licensed veterinarian.
My puppies are sold as pets only with limited registration and a spay or neuter
contract. Registration papers are given as a courtesy only on pet puppies.
There may be exceptions to this rule, that would be determined on an
individual basis. Pups bought for breeding or show would be sold at a higher
price, not the pet puppy price. Indian Creek also guarantees the direct
parentage of a pup sold off my property (dam and sire only). Small hernias will
not be repaired except they are pronounced and the dog is going to be used
for breeding purposes. Both the buyer and the seller of the puppy hereby
understand that the buyer is responsible for any and all veterinary costs of
every kind and nature after the date of purchase. No other guarantee are made
or implied other than those stated in this written health guarantee. This
guarantee must be signed and mailed or faxed back before a puppy will be
shipped.
Seller assumes no responsibility or financial liability for any veterinary visits or
treatments.
I the purchaser of a puppy from Indian Creek terms and conditions of the
guarantee and agree to have puppy spayed or neutered within four months
from date or purchase, unless agreed upon otherwise. Failure to send a copy
of proof of spay or neuter and current vaccination papers to Indian Creek will

must be received within 72 hours or puppy goes back up for adoption
